Just before we left, two sisters living just an hour away were diagnosed with another form of Batten disease. Lilly was six then. She was running around, playing with friends, and singing her favorite songs. Today, Lilly is blind and spends most of her time on the floor. So was her little sister Lilly. At the time, there were four promising treatments in the pipeline. There was hope. But despite the explosion of scientific advancements over the past few years, each of these treatments have been shelved or paused. Today, Cecilia has lost her vision, her mobility and her independence. Just a few last words remain. As she sits in her dark world, her father closes his eyes to join her, his hands in hers. He was never given the chance to try a treatment for his girls. Like Mila, she was born at the end of 2010, had thick silky brown hair, big brown eyes and a mischievous laugh. Like Mila, I watched as she zipped around on her scooter, dove into the pool and smiled wide for the camera. Like Mila, she had taken a mysterious turn and began a rapid decline. And then she and her little sister Gwenyth were diagnosed with the fatal condition, Batten disease. Just days after watching the video, Mila too was diagnosed with the same devastating disease. Both girls were destined to die. Charlotte's parents raced against time to stop their girls' disease and started one of the very first gene therapies to the brain. Because of their help and friendship, I was able to fight for Mila. Both girls were fortunate enough to be among the few who received treatment. But too many dominoes had fallen by then. They would become the pioneers who are helping those who follow. A few weeks ago, Charlotte turned 13. She smiles as her mother gently, but firmly supports her growing body as she takes slow steps toward the living room where her younger brothers await. Gwen was just 3 when she received one of the first ever gene therapies to the brain which her parents fought day and night to start. Today, Gwen is in a place far better than where she should be given her disease. Fourteen other children with Batten CLN6 were part of her trial, which a company took over in order to commercialize it and make it accessible to many more. But the regulatory pathway and financial system weren't designed for biotechs pushing treatments for the small populations of rare disease. Like other new mothers, my time and energy moved away from taking care of myself to taking care of my baby. I nursed her, I bathed her, I rocked her to sleep. Then she grew and became more independent. She could walk and run and navigate the house on her own. She spoke and told me when she was hungry, tired or sick. She could feed herself, sit on the toilet, climb into her bed and fall asleep alone. But then, around four years old, she started to regress. Just as I returned to holding Mila's hand again when she walked, cutting up her food into small pieces again and sitting her in a stroller to get through the grocery store, Kristen Gray was doing the same for her first child, Charlotte. Neither of us understood why our daughters were going in reverse. There was no taking care of ourselves. Our girls depended on us more and more by the month. Our answers came when both of our young daughters were diagnosed with Batten disease and we were told that all they learned they would soon forget. But I stuffed that yearning into a box, closed it and pushed it to the back of a dark closet so I could fight for Mila. It was out of sight, but never gone. Toward the end of Mila's life, I reached back into the closet, opened the box and pulled out the longing, burning stronger than ever. Perhaps I wanted another child to fill the hole in my heart that was coming. Perhaps to ensure that Azlan wouldn't be left alone in this world, carrying the pain of what he had lived through. Perhaps to try to bring Mila back. For better or worse, life as a single mom did not allow me to live my dream. But for parents like me, this decision often comes at an age and time that make having another child seemingly impossible. Like me, Kristen Gray and her husband navigated this difficult path while at the same time fighting for Charlotte and Gwenyth, their only two children, both with a disease no child had ever survived. When the girls were 6 and 8 years old, they finally had a healthy little boy named Cal, and a year later another, Gavin. A new life of immense joy with the backdrop of immense sadness. Unlike Mila, Gwenyth Gray still has her vision and occasionally climbs out of bed and crawls down the hallway. She wakes up in the morning and opens her eyes to sunlight and the room around her. Gwen received one of the first gene therapies to the brain when she was just 3 years old, and 4 years later began taking a promising off-label medication. She is able to watch cartoons, use her hands to feed herself and interact with her little brothers. She smiles and she laughs. In these early years of learning from genetic therapies, those fortunate enough to access a treatment are often searching for other complementary approaches. A decision no parent should ever have to make. Like me, Kristen Gray finds herself in the impossible place of having to think for her daughter Charlotte, who has lost nearly all of her abilities and can no longer tell her mother how she feels. Sleeping with her one-year-old son on the sofa just outside Char’s room, Kristen jumps up and rushes to her daughter’s side numerous times every night to suction the saliva from her mouth and ease her back to sleep. Each day, despite little to no sleep, Kristen and her husband get back up on their feet and face a full day of school drop-offs and pick-ups, therapies, doctors appointments, and jobs. They do it because they must. I met them, together with their brother and parents Trisha and Chris, on Zoom when the girls were tiny, soon after they were diagnosed with SPTAN1, a rare genetic disease. At the very dawn of their lives, their fate was handed to their family on a plate - die young or survive, trapped in the minds and bodies of infants whose breathing, eating, sleeping and moving would forever depend on others. The trajectory of Trisha and Chris' lives is far from what they had dreamed of, at first glance unbearable even to me after what I have lived with Mila. Every moment of every day and every night consumes them. Trisha and Chris Lockard face this for both of their girls, Iris and Logan. They trade off throughout each day and night handling feeding tubes, oxygen machines, seizure monitors and mucus suctions. There is no room for error. They carefully hold their girls, dress them, feed them, put their leg braces on, and soothe them back to sleep. He was 3 years old when his sisters were born and soon after diagnosed with a rare genetic disease. He leans back and watches as his parents prepare meals and medications, give oxygen, and dress, feed and position his sisters in their therapy equipment. Jude may never remember life as a typical kid with a typical family. His life is different now, but that's all he knows. He loves his sisters and wants them nearby. His parents want him to feel part of a family, like his friends do, so every few weeks they rally and bring their girls to his soccer game, packing up three children, a double stroller, bag of diapers, water, food, medicines, breathing machines and neck pillows, then unpacking them all when they arrive, carefully positioning the girls and making their way to the field. But for families with children like Logan, Iris and Mila, it's the tiniest moments that bring us the most happiness. A small clenched fist that reaches out for connection, a possible intentional smile, little eyes intensely locked with our own, sending silent love through the air. It suddenly seemed miraculous that people all around me were capable of lifting their feet to walk, reaching out their arms to grab things, driving cars and navigating busy streets, and feeding themselves by chewing and swallowing pieces of carefully cut food. For parents like us, Trisha and Chris must take over where Logan and Iris' bodies and brains can't. They pump oxygen into little mouths, strap small bodies into devices to help blood circulate, open tightly clenched hands and massage fingers, and sit both girls in their feeding chairs, tilted back with supportive neck pillows, carefully spooning warm pureed food into their hungry mouths. All of this because of a little mutation in the wrong place. He understands everything other 4 year olds can, but his genetic disease, alternating hemiplegia of childhood, or AHC, has made life far more difficult, frustrating and scary than for the other kids his age. Like Henry, Mila struggled to tell us what she wanted to say. I can still feel the sadness of watching her try so hard to form the words, only to sigh, bow her head and give up. Henry’s parents find their way of connecting. They sit beside him and say, "how much do we love you?", waiting in silence as Henry responds by reaching one arm up high to say "Soooo much!!". On top of that, every month an even bigger, terrifying one hits unexpectedly and lasts 15 minutes, sometimes more, requiring resuscitation and ambulances. Each time, his parents beg the universe not to take Henry, not this time, to give him just a little longer to make it to a treatment that scientists are racing to develop and get to him. In just the last year, two of the few children in the world with Henry's mutation have died. His parents haven't told him, but he senses their fears, crawls over and kisses them as if to say "don't worry, I'll be ok." Before I watched Mila lose her ability to run, then walk, I had never realized how precisely numerous different functions had to come together for a person to take just one step. For Henry, it takes everything in him to walk just a few feet from the playground slide to the bench. He wants to be independent like other 4 year olds so he tries, but he knows he may fall. He fits perfectly in Henry's little hand. Lion goes everywhere with Henry. He eats Henry's snacks, takes his medicines before he does, and if Henry isn't thirsty, but Lion is, Henry takes a sip. Henry's disease, AHC, may have robbed him of much of his mobility and speech and threatened him with seizures, but playing with Lion makes him feel like just any other kid. While fighting for a treatment for Henry, his parents make time for these little moments. They walk the fine line of balancing the future and the now. They watch as Henry sits on the floor, moving his body as he can, creating an imaginary story around Lion. They feel Lion's excitement through his sounds, see Lion running through Henry's quickly moving legs. As Henry plays, I can't help but think of Mila smiling so widely, her fingers wrapped around her snowman Olaf, lifting him and dropping him, the only way she had left to play. Each time a big one comes on, the fear is the same. It's just another day until time stops, just like that, and a seizure runs through Henry's brain like a storm. His parents, Mary and Anthony, drop the blocks they were building with Henry and snap into clinical mode, urgently grabbing the rescue medicine from Henry's bag, injecting it into the side of his mouth, laying him on the floor and setting the phone beside his limp body watching the clock, mentally calculating how many more minutes before they call 911 and what needs to be packed before the ambulance arrives. On a lucky day, the seizure ends at home. But on the worst days, they clench their hands together in desperation and silent prayers as Henry is rushed to the hospital, not knowing if he will return. At the end of these days, Henry falls into a deep sleep from the devastating seizures and medicines that have rushed through him. A dad and mom sharing the excitement of their daughter, little hands grasping a new toy, eyes wide with curiosity at the tiny image on the camera screen. What you can't see is that this little girl, Ipek, has a fatal genetic disease - ataxia telangiectasia, or AT - which eventually robs children of their ability to read and write, slurs their speech, confines them to wheelchairs and makes them highly likely to get regular lung infections that don't respond to antibiotics and malignant cancers like lymphoma and leukemia. Children with AT often don't survive their teenage years, at most into their twenties. But Ipek received a highly targeted genetic treatment at just three years old and became the second child after Mila to receive an individualized medicine. Four years have gone by, and although regular injections and hospital monitoring are the price to pay, Ipek's life is more normal than what it should be. Facing a rapidly progressive fatal condition, a longer life was the obvious answer. But when I dove deeper beyond time, I found my answers in happiness and interaction with the world around her. A few years after Mila began receiving her treatment, Ipek followed with her own individualized medicine, but she was treated earlier on in her disease. Only time will tell if a longer life lies ahead, but in the meantime I can't help but notice her joy and the friends who surround her at swim class, a red-haired mermaid clutched tightly in her hand. I didn't know that Mila's ease in placing little people into her doll house would come to an end, childhood laughter turning into exhaustion and confusion. Unlike Mila, Ipek was treated early on in her disease. One big step towards the future of finding and treating children at birth before symptoms even begin. In many ways, Ipek’s life is looking better than where it should be. But it isn't black and white. Ipek was two years old and had just started walking. Mila was nine and her legs were just starting to give out. Ipek walked beside Mila, supported by me from behind, with Mila's birthday balloon in her hand. That day, I remember thinking that Mila had given her legs and likely her life to Ipek. Learning from one little girl to help the next. When he was less than a month old, his parents were told he would lose his vision, memory, mobility and life. Wheeler has CLN3 juvenile Batten disease, a condition that, on top of losing nearly all of his abilities, causes him mental destruction and behavioral storms, and eventually will rob him of his life, likely in his teens. Getting through an average day is exasperating for everyone in the family. Wheeler is both sensory sensitive and sensory seeking, making even the smallest of tasks - like a few bites of dinner- challenging. His mom Judy plays ‘knights and dragons’, making growling sounds as she hangs her 50lb son upside down, struggling to hold up his weight, to meet his sensory needs in the hope he will sit down and eat his dinner. Meanwhile, Wheeler's dad Court plays with his younger son Read, who is not affected by Batten, but like any child, has his own wants and needs - needs that are hard to meet in a home that revolves around Wheeler. Even their two dogs chip in to support, often approaching Read with a ball to play with or lying on Wheeler to provide sensory input. Court and Judy divide and conquer, fighting their exhaustion from a long day, in hopes of being the best parents they can be amidst the overwhelming situation they were dealt. It's just one of the many therapies Wheeler takes part in every week, but it's the most unique. Today, it started with the hat he took from a friend and his mom's calm explanation of why he needed to give it back. But she knows the scream runs deeper. When she reached out to me at that time, I remember thinking, if any child has a chance at beating the odds, it's this little boy. But five years later, all 3 treatments have been stalled, shelved or abandoned, leaving Wheeler and kids like him in limbo between bureaucracy and the promise of scientific innovation. And now his symptoms are taking over. Wheeler’s parents have found themselves fighting a system of access to therapeutics not designed for their son, while at the same time fighting a world not fit for him. Screaming fits, negotiations, calming, anger, frustration, exhaustion. She loves baby dolls, painting and riding her bike. But she has a rare neurological disorder called Alternating Hemiplegia of Childhood (AHC) which causes episodes of paralysis, limpness and an inability to respond normally to life around her. Children with AHC can experience these symptoms for hours or even days on end, with each episode causing possible irreversible brain damage. Annabel is in a period of calm. She doesn't experience seizures, many of her symptoms have been stable over the last 6 months and she is able to attend a typical school. Her parents' time and energy go to caring for her and giving her the attention she needs, while at the same time fighting to fix her genetic condition. Her mother Nina stays up until 2am, putting in the hours to work toward a treatment, then wakes at 6am to get Annabel up and ready for school. But perhaps the hardest thing Nina and her husband Simon face is the guilt and sadness of giving so much of themselves to Annabel and worrying that there isn't enough to give to their older daughter, Clara. This same guilt weighed heavily on me every day as I carried Mila around the house, spoon fed her food and helped her go to the bathroom, with Azlan in the room next door, alone, playing with his toys. On top of the loneliness, Clara watched her grandmother decline and die of cancer, all while watching her sister have episodes and need rescue medicines. For Clara, growing up like her friends isn't an option. Raiden's parents, Linda and Tommy Pham, wake up every day and balance surviving with racing against time for a treatment that could save their son's life. Like in the case of thousands of rare genetic conditions in children, the only hope for Raiden lies in the hands of his parents. But to make it to a treatment requires more than the already impossible task of raising millions of dollars and putting scientists together. It means keeping Raiden strong enough through a regimented routine of meals, water, medicines, exercises and machines, and up to 5 therapies per week during working hours. When she was 3, she was diagnosed with HNRNPH2, a rare condition with a bleak and unknown future. Today, Rosie is 9. She spends most of her time at her dad's house, her mom's, and her small school, as loud and busy places normal families would go are too overwhelming for her. She wanders the house in silence, picking up stuffies and jewelry in her sister's room, opening living room drawers and removing the contents, and taking eggs from the fridge and throwing them on the floor. Casey has spent the last 4 years fighting for a cutting-edge genetic treatment for Rose in hopes of bringing back even a few words, or giving her the ability to wind a toy on her own or sit on the toilet. But what he's fighting for most is his daughter's ability to connect with the world around her. He watches her stand alone on a playground longing for interaction with the kids who run past her. And he sees how she fills that void with animals who connect without words and do not judge her. And so he has lived in the country where he can surround Rosie with pigs, goats, chickens and dogs. And if she does, who will take care of her when her parents are gone? These are the uncertainties that drove Casey to leave his music career, the one he worked on day and night for 25 years, to fight for his daughter. They were given the gift of more time, but the unfathomable understanding of what's happening to their bodies and brains. Both siblings receive a regular enzyme replacement therapy which offers hope of slowing their condition, but Maya is older and started when her disease had already begun to take over. Xavier is mostly stable for now, but faces the heavy weight of his own mortality and that of his sister. Her family tries to live a normal life, but big seizures hit when they least expect it, turning a short walk to the playground or a drive to the beach into a medical emergency that risks Amara's life. When the rescue medicines fail to work, the ambulance arrives. She tries to make friends at school or on a playground, starting with soft noises and giggles, often staring at them for a response or even reaching to them for that touch and connection. If Quincy is at home with a sitter, they know she is loved and happy, but the guilt is just too much.
